No Show & Cancellation Policy

Dear valued Animal Medical Hospital clients,

We would like to inform you of a new policy on cancellation of appointments and no shows. This policy was established to provide the highest level of veterinary care to all of our patients.

We understand that sometimes you need to cancel or reschedule your appointment. However, when you do not call, in advance to cancel an appointment, you may be preventing another pet from getting much needed treatment. It is the responsibility of the client to arrive for their appointment on time and cancel, if needed with at least 2 hours’ notice. Clients can leave a voice message if the lines are busy at 812-663-7171, or they can respond via text to the reminder message sent.

You will be documented in your record as a “No show” appointment if:

1. You will cancel with less than 2 hours’ notice,

2. Fail to show up for your appointment,

3. Arrive more than 15 minutes late for your appointment without prior notice.

No show penalties for current clients:

A $50.00 fee that will need paid in order to further schedule.

For all new patient appointments: we will collect a non-refundable credit card payment equal to the initial consult fee when scheduling the appointment. If you are present for your first appointment, the amount will appear as a credit on your invoice toward the visit.

No show penalties for new clients:

If you fail to show up or fail to give the required 2 hours’ notice for cancellation/rescheduling the appointment, the initial consult fee will not be refunded or transferred. You will receive a phone call stating you have been counted as a “No Show.” The veterinarian you were scheduled to see will decide if they will accept you as a client.

For scheduled surgeries, a $50 dollar down payment will be required at the time of scheduling. Clients who fail to cancel or reschedule within 1 business day will be considered a “NO Show.” The client will forfeit their deposit.

Note that all fees incurred are not covered by any pet insurance you may have and must be paid prior to another appointment being scheduled.

A copy of the letter and any decisions made by the veterinarian will be placed in your record.
